REGISTRATION FORM

Instructions for filling Online Registration Form:

1. This form works best with Firefox and Google Chrome web browsers.
2. As this form will be using internet, in case of slow internet, the process will be slow. Do not refresh the page or open it in a new tab while filling up the form.
3. Fill basic details in Registration Form. You will receive Username and Password on registered SMS Number.
4. Log In with Username and Password (Both are case-sensitive).
5. Enter remaining details. Download the form and attach a copy of supporting documents. Do not attach original documents. The Original Certificates must be produced at the time of Admission.

Admission Document Requirements

1) Nursery to 1st Std.

Birth Certificate: A document that verifies the child's name, birthdate, and parents' names issued by the Municipal Department/Block/Hospital/Town Council (Nagar Panchayat).
Residence Proof: A document that verifies the child's and parents' address.
Passport-sized Photos: 2 Passport size photographs of the child.
Aadhar: Xerox of Aadhar Card of the child, mother, and father.
Blood Group: A document that verifies the child's health and absence of infectious diseases.
Medical Certificate: A medical or immunization certificate issued by a registered medical practitioner.
Caste Certificate: A document that may be required for the reservation category if it belongs to SC/ST/OBC categories issued by the Sub-District Magistrate for admission.

2) For Class - 2nd to 9th and 11th Std.

Transfer Certificate (T.C.): The original transfer certificate from the last school attended.
Residence Proof: A document that verifies the child's and parents' address.
